Peer Support

  • A firsthand guide is a peer who understands and has lived experience with serious mental illness and/or substance use disorder. They have worked through these challenges in their own lives. They provide personal, hands-on support to each individual - actively listening to discover, understand and address each person’s self-perceived needs.

Social Support

  • A Community Resources Guide understands and can connect an individual to benefits that they may be eligible for in their local community. Our Community Resources Guides can help with housing, food, utilities and more.

Health & Medical Support

  • A Health Guide is a clinician who will take ample time to assess and address all of an individual’s unique physical and mental health needs, both urgent and longstanding. Health Guides provide referrals to trusted partners for additional care needs and can support with treatment plans and medication adherence.

Our team is local. We know your community and neighborhood. We know what local services are available and how to make sure you can access these services.

We can meet you in your own home, a local clinic, or any other convenient location and help you get support to improve your health and wellbeing.
